Experienced team is in place with the CEO, Chimezie Onyima who has 12 years experience in supervising and delivering quality health care services to individuals with developmental, cognitive and physical disabilities, as well as Challenging behaviors.
We (Chimezie Onyima and Ijeoma Obikaonu) have the ability to maintain staff training records. We are familiar with completing monthly review of individual plan/goals and progress report, ensuring all routine and non-routine participant’s medical appointments are scheduled, making sure all medical appointment sheets are completed and signed by the physician and following policy in relation to medical appointments.
All staff will be required to successfully complete the DDA required trainings as specified in Training Requirements by position before or within 60 days of hire. Staff will also successfully complete training in specific areas of need for the individuals they work with, as identified in the person’s person-centered plan. Transportation is provided to enable individuals to live meaningful life in the community. Staff that transport individuals who receive supports complete a defensive driving course. Additionally, the motor vehicle records of all agency drivers are obtained upon hire and as needed and/or recommended by the agency insurance carrier.
Also, Trinity Life Care Inc. ’s Staff will successfully complete any additional training including services as needed in the areas that will support the individuals serve when they are employed. Staff will be able to assist in areas that are identified in the person’s Person-Centered Plan. Every Training is individual specific, and staff must have knowledge and know that they share the philosophy of all “people no matter the race, religion, or disability should be involved with their communities. The Program Coordinator in concurrence with the HR will work with the house managers to oversee when certifications are expiring and communicate with staff 8 weeks prior to expiration date, to get staff current on all renewals. In addition, as a part of our team the staffing plan shall include on-site staff sufficient in number and qualifications to meet the 24-hour scheduled and unscheduled needs of the participants being served.

Nursing will oversee and monitor CMT’s ta make sure that they are:
- Current and up to date with their license to distribute medications
- All staff are trained regarding the medications the individuals in the residential or day setting are prescribed
- Make sure that PMOF’s MAR’s and Prescriptions are current
- Provide disciplinary actions when needed regarding staff making medication errors or mishandling medication

All services will be provided in the following counties: Howard, Anne Arundel, Baltimore County, Montgomery County and Prince George’s County.
